Is It Sciatica or Something Else? Understanding Referred Leg Pain and Low Back Symptoms

Leg pain originating from the back is a common complaint, but not all leg pain is sciatica. Distinguishing between true radicular pain (sciatica) and referred pain is essential for determining the appropriate management approach. Most cases of both conditions respond well to conservative care, though understanding the underlying cause is key to recovery.

Many people experiencing discomfort that travels from the low back into the leg assume they have sciatica. However, the term 'sciatica' is often used as a catch-all for any leg pain, even when the cause is different. Understanding the distinction between these symptoms is a primary step in the assessment and conservative management of low back pain and sciatica in Richmond Hill. Referred pain occurs when structures in the spine, such as joints or muscles, cause pain to be felt in the extremities [14]. Unlike sciatica, which is typically caused by nerve root compression, referred pain is often described as a dull, achy sensation that may move around and rarely travels below the knee [15, 18]. In contrast, true sciatica—or lumbar radiculopathy—is characterized by pain that radiates down the leg, often below the knee, and may be accompanied by numbness, tingling, or weakness [11, 17]. This occurs when a nerve root in the lower spine is irritated or compressed, often by a herniated disc or spinal stenosis [9, 19]. Current evidence suggests that the majority of sciatica cases improve within six to eight weeks with conservative treatment [3, 9]. Because the body can often reabsorb the disc material causing nerve irritation, time and guided activity are frequently the most effective tools for recovery [17, 24]. Contributing factors to both conditions include prolonged sitting, poor posture, and reduced core strength, which can place increased stress on the lumbar spine [12]. An assessment at our clinic may include a review of your health history, an evaluation of your posture, range of motion, and a neurological exam to assess reflexes, muscle strength, and nerve function [22]. Conservative management options are tailored to the individual and the stage of the condition [25]. For acute stages, this may focus on patient education, activity modification, and gentle movement [19, 25]. As symptoms stabilize, care may incorporate strength training and, in some cases, spinal manipulative therapy to help restore functional capacity [25]. While most cases resolve naturally, it is important to monitor for 'red flags' that require prompt medical attention [10]. Seek immediate care if you experience loss of bowel or bladder control, sudden or severe weakness in the legs, or numbness in the 'saddle' area [9, 12]. If your symptoms are persistent, worsening, or significantly disrupting your daily routine, a clinical assessment can help clarify the contributing factors [9].
